1. All patients must wear a mask or cloth covering over their mouth and nose.
2. No companions are permitted to remain in the treatment rooms during treatment. They may assist with shoes and sock removal and then have a seat in the waiting room.
3. All visitors will submit to a temperature screening upon arrival; further access will be denied to anyone whose temperature is greater than 99.5 degrees Fahrenheit.
4. New Patients: All New Patient Paperwork will be completed on the form link below.
5. If you (or your companion) have symptoms of
a. Fever, cough, shortness of breath, sore throat, muscle fatigue, vomiting, diarrhea, abdominal pain or lack of appetite,
OR
b. If you or anyone in your household has been tested for the coronavirus or tested positive for the virus.
