New Policies and Procedures Related to Covid-19
1. All new patients must be fully vaccinated and wear a medical grade mask over their mouth and nose. A cloth mask is not acceptable at this time.
2. Companions are permitted to remain in the treatment rooms to assist with shoe and sock as long as they are vaccinated and masked appropriately as well.
3. New Patients: All New Patient Paperwork will be completed on the form link above.
4. If you (or your companion) have symptoms of
a. Fever, cough, shortness of breath, sore throat, muscle fatigue, vomiting, diarrhea, abdominal pain or lack of appetite,
OR
b. If you or anyone in your household has been tested for the coronavirus or tested positive for the virus.
